Gather The Following Documents:
Paystubs
If Paid weekly: gather last 4 paystubs
If paid biweekly: gather last 2 paystubs
If you do not have paystubs: Download and have your employer fill out the Verification of Employment Income Form. You can also download the Employer Letter which contains instructions.
ID
Any state issued, picture ID of all adult applicants
Birth Certificates
Copies of birth certificates is required for all children in the household, whether they are the child in need of childcare or not. This is required to prove household composition size.

Social Security Cards
Copies of all available Social Security cards for the children only. The adult applicants do not have to supply their social security cards.
